ALL FORMS MUST BE COMPLETED TO PLAY VOLLEYBALL THIS SEASON

1) Sign up for 2012 USA Volleyball – go to www.usavfl.org and either renew or register for your child. Print out the registration card as proof of membership and bring to the tryouts.

2) Sign Up for 2012 AAU Athlete Membership - Click here to Join Now

Make sure to choose Suncoast as your club (Club Code: TF485A). Print out the registration card as proof of membership and bring to the tryouts. If you are already a member of AAU (2012 Season) you do not need to register again.

3) Copy of the Player's Birth Certificate - Driver's License or Passport

4) Fill out the following 2 forms and bring to the tryouts with your child’s birth certificate, driver's license and/or passport as well as the USA Volleyball Membership Card and AAU membership Card. There will be a Notary at the tryouts.

5) Pay $55.00 Registration Fee either online or by mail
